    The NIHGPS is the official document that outlines the policy requirements, terms, and conditions for all NIH grant and cooperative agreement awards. Updated annually to reflect changes in statutes, regulations, and NIH policies.
  • Notices of NIH Policy Changes (Visit website)
    Policy notices published in the NIH Guide for Grants and Contracts will supersede information in the NIH Grants Policy Statement. NIH incorporates these notices into the annual update of the NIH Grants Policy Statement.
  • Uniform Guidance 2 CFR Part 200 (Visit website)
    Uniform Guidance refers to the federal regulations (codified in 2 CFR Part 200) for managing U.S. federal awards, which covers administrative requirements, cost principles, and audit requirements for non-federal entities, such as nonprofit organizations and institutions of higher education.

  • Investigator Significant Financial Interest Disclosure Policy Acknowledgement Form (PDF)
    One-time acknowledgment that you have read, understood, and agree to abide by the current FCOI policy.
  • Financial Disclosure Form (PDF)*
    Required form for all investigators to disclose significant financial interests for each study. Required annually or when there is a gain of significant financial interests.
  • QHS Intranet (Visit website)*
    Visit the QHS Intranet for a repository of administrative forms for Finance, HR, Purchasing, and many other departments.
