Research Allied Health Professionals (RAHP)

Research Allied Health Professional (RAHP) status must be obtained before a non-QMC employee interacts with patients or accesses patient information at The Queen's Medical Center.

Instructions for Applications

  1. Download, read, and print a copy of Allied Health Checklist, and keep for your records.
  2. Download and complete the Research Allied Health Professional application and Sponsorship of Allied Health Professional forms.
  3. Please mail OR e-mail all original, completed, and signed forms.

If you are a Non-QMC Nurse Researcher, you will still need the documentation listed here, however please coordinate all your application documentation, approvals, and authority implementation with:
